All information shared with our clinical staff is confidential and data is protected under the data protection act.
Respect for all religious and cultural beliefs will be honoured in our surgery.
We will endeavour to include you in decision making about your health, we will offer health promotion advice and help you promote your health.
You have the right to see your own medical records subject to the limitations of the law. Please be aware a charge may be required.
We appreciate your constructive comments on how we can continue to improve our services; we will take your comments into account to enable us to ensure our service is tailored to our specific patient's needs.
Patients' Obligations, Rights & Responsibilities
Please let us know as soon as you change any of you contact details so that we can update our records. If you do not inform of any changes to your contact details at the earliest opportunity there is a risk that you will miss important information about your care such as hospital appointments or test results, or that information about you may be received by someone else.
Please let us know promptly if you cannot attend an appointment so we can offer that slot to another patient.
You are responsible for your health and that of your children; please take the medical advice given to you by your doctor or nurse.
You have the right to a second opinion and can discuss any matter with our doctors.
