Online Services & Online Access
Last Updated: Friday, 15 March 2024You can set up an online account to:
- Order repeat prescriptions
- Nominate a pharmacy to collect your prescriptions from
- Book, check or cancel some appointments with a Nurse or GP
- See parts of your medical record such as test results and vaccinations
- Receive messages from your GP practice. You must switch on NHS App Notifications to do this.
You can register for an NHS online account online by clicking the button below.
Alternatively, print, complete and return the Online Access form below. We will need you to show ID (please see form for examples) when you return your form. We will then give you a code and instructions to set up your account.
Updates to online access
On 4th October 2023 the things you can see in your online account changed. This update was applied to people aged 16 and older.
If you had an Online Account before 4th October 2023
You will now be able to see more information added to your record after 4th October 2023. This includes:
- Consultations
- Documents
- Test Results
If you set up your online account or joined Sefton Park Medical Centre after 4th October 2023
You will be able to see information added to your record after the date you set up your account or joined the practice. This includes:
- Consultations
- Documents
- Test Results
Important: If you do not want to see certain information in your online account
You may not want to see your consultations, documents or test results in your online account. We can switch off any or all of these features if you ask us to. You can request this by speaking to our Reception Team (at the surgery or call 0151 295 8700). You can also email your request to changemydetails.seftonpark@nhs.net

Accessing someone else’s information
As a parent, family member or carer, you may be able to access services for someone else. This is called having proxy access. We can set this up for you if you are both registered with us.
You can apply online if you would like to help another adult to manage their online services, or choose someone to help manage yours. This service is designed and run by the NHS, so your personal information is safe.
You can also apply online to manage the online services of children under 16. You must be the parent or guardian with legal parental responsibility to do this. This service is designed and run by the NHS, so your personal information is safe.
If you would prefer to apply using a paper form:
- Read one of the information leaflets below (either child or adult leaflet)
Or
- Bring your completed form to the surgery along with your ID between 10am and 6pm Monday to Friday.
If you are not able to print a form at home you can collect a paper proxy access form from Reception.
Important: Returning your Proxy Access Form
Both the person whose account is to be accessed (if they have capacity) and the Proxy will need to attend with ID.
