Updating Your Personal Details or Contact Details

It is important that you let us know about any changes to your contact details, address or personal details. Examples of this are:

• Change of phone number (landline or mobile)
• Change of address
• Change of email address
• Change of name

If we don't have your up-to-date details you may miss important information about your care. There is also a risk that information about you may be received by someone else.

To update your details complete the form at the bottom of the page.

Or you can email us at changemydetails.seftonpark@nhs.net including:

• Your full name
• Date of birth
• Old details
• New details

If you believe we have your Date of Birth recorded incorrectly let us know. You will need to provide proof of your correct date of birth so we can change this for you.
